Levothyroxine is the second most commonly prescribed medication among older adults in the U.S. However, researchers have noticed that many people taking levothyroxine might not even have hypothyroidism.

In these cases, the medication could cause high thyroid hormone levels, which have been linked to higher risks of fractures and bone density loss. This led scientists to ask a critical question: Could levothyroxine, even at recommended doses, harm bone health over time?

This research, conducted at Johns Hopkins University, used data from the Baltimore Longitudinal Study of Aging – a long-running study tracking the health of older adults living independently in the community.

Researchers focused on participants aged 65 and older with normal thyroid function and at least two visits with consistent thyroid function test results.

They compared these 81 levothyroxine users with 364 non-users. Both groups were carefully matched for age, gender, body mass index (BMI), race, smoking and alcohol use history, and other factors.

Bone health was assessed using specialized scans called Dual-Energy X-ray Absorptiometry over a median period of 6.3 years.

The study’s findings were alarming.

1. Levothyroxine users experienced a significantly greater total body bone mass loss than non-users.

2. Bone density declined faster in levothyroxine users.

3. The effect was stronger in participants with higher levels of free thyroxine (FT4) yet within the normal range.

This is terrible. This means levothyroxine use is linked to greater bone loss over time, even in people whose thyroid glands function normally.
